Urgent Care VisitBook this visit for issues that require immediate attention like when your asthma is acting up, or when you have a UTI, an ear infection, or pink eye.

General Medical VisitThe General Medical Visit is a perfect choice for when you want to address just about any medical complaint. Use this option to refill medication, address your blood pressure, etc.

Administrative Evaluations   (In-Office)

Immigration Medical ExamWe are designated Civil Surgeons. Book this appointment to start your USCIS Medical Evaluation and documentation of the i-693 form.

Pre-Surgical Evaluationf you’re about to have surgery, your surgeon may require a physical exam to make sure you’re are healthy for general anesthesia. Check with your surgeon before booking.

Sports Physical ‘Tis the season for sports and you need a doctor for clearance. Use this option to book a visit. Be sure to bring your form with you.

Disability Physical ExamChoose this exam to book a DSS, EDD, FMLA or other disability evaluation. If you were given any forms from your work or other entity, please be sure to bring it with you.

Employment Physical This exam is designed to give a general medical evaluation as requested by your employer. If there are any forms to complete, please be sure to bring them.

DOT Commercial Driver We are certified by FMSCA to conduct DOT/DMV Commercial Driver medical evaluations. Please allow up to 1 hour for this exam including completion of the MCSA-5875 form.

School Health ExamThis exam is designed to give a general medical evaluation as requested by your school. If there are any forms to complete, please be sure to bring them with you.

COVID-19 TestingChoose this appointment for nasopharyngeal swab testing to detect COVID-19 infection.

COVID-19 Vaccine Choose this appointment to start or continue your COVID-19 vaccine series or get your booster. Please be sure to complete the Adult Vaccination Form prior to your vaccination.

Kraft Test (Insulin Resistance)Book this appointment to see how close you are to developing diabetes. Please allow 4 hours for this test. 12-hour fasting required prior to testing.

Flu Vaccine Book your flu vaccination between October 1 and April 30 every season. Please be sure to complete the Adult Vaccination Form prior to your vaccination.
